First, I want to apologize if this has been answered in another thread.
I followed the directions from here to fix the problem. I just wanted to know why this little bug keeps reoccurring. I don't duplicate my internal drive or keep copies of apps stored eternally as was mentioned in the link. As far as I know this has been happening since OS X Lion when I first purchased the mac. I know its a small problem to complain about, but sometimes the list can get ridiculously large and makes the quick "open with" option inconvenient to use.

Here is an example of the problem starting to build up only an hour after I reset the "open with" menu:

I find it is caused by bad app installers. Just my opinion. I don't think it is anything you are doing wrong.

Funny you have Pixelmator installed. I noticed my launch services was jacked up a week or so ago with the last Pixelmator update. The Termnal command you mentioned fixed it.

Launch Terminal, located at /Applications/ Utilities/

Copy and paste the following command into the terminal and press return.

/System/Library/Frameworks/CoreServices.framework/Frameworks/LaunchServices.framework/Support/lsregister -kill -r -domain local -domain system -domain user​

Give it a minute or so, then Option + Right Click your Finder icon in the task bar and select Relaunch

Should fix the issue. I just used this last week to clean my list up.
