OSXWiFi installation instructions?

A few years ago, I ordered a 802.11ac/BT 4.0 kit from OSXWiFi for my Mac Pro 4,1. Unfortunately, the Mac Pro died recently. I did, however, find a nice 5,1 as a replacement.

So I lost the installation instructions for the original kit, and I've been trying to contact them to get a copy, but no response so far.

Does anyone maybe have a copy of the installation instructions for a 4,1/5,1?
