I’m not sure if it is related to the security recommendations or not but I have about 15 passwords at yyy.com and all of them are old. So I go through and delete them in various ways. One is I search for yyy.com and then swipe left and hit delete. Another is to go into the Recommendations section and delete it from there. In both cases, on iPadOS and iOS, the passwords keep coming back.

How can I stop that from happening?
 
Where are you deleting them? Where are you searching for yyy.com?
On iOS and iPadOS, in Settings => Passwords, there is a search option. On macOS, in Safari, there is Preferences => Passwords that also has a search option.

I'm deleting them because they are not valid. The account was destroyed long ago and they are to systems within yyy.com that are gone.

I've deleted them now from all three places and they still come back.

There are also websites that no longer exist like discovercard.com. It redirects you to discover.com where I’ve updated my password but the entry in Passwords still exists. I try to delete it but it comes back. All in all, the system of Recommendations seems to be half baked so far.

On macOS look in Keychain Access in Utilities and delete them there.
iCloud Keychain is probably restoring them after they're deleted.

Yea. I tried that too. I also tried logging out of all of my devices except my Mac, delete them from the Keychain Access, log out, log back in, waited 15 minutes, then looked and they were back. Something is horribly broken. I’m going to just ignore them for now.
