Phone app crashes every time

I am unable to use the Phone app to place outgoing calls on my iphone 5s. As soon as I open it, it crashes. If I restart my phone, it will work for a little while and then it will start crashing again. I have no problem receiving calls. Anyone have any ideas?

Restore the iphone with itunes.
Setup as new, not from backup.
Is your phone JB?
