Hi...I have 20k+ pics and I'm crazy OCD so I have a very extensive file system that I keep them in. I also have backups on both dropbox and iCloud drive. I have been manually updating both each time I put new photos into my picture file system on my iMac, but I was wondering if there was an app or process that I can get to automate this process for me? Something specifically that will recognize changes to Pictures (including sub folders) and copy and paste the "changes" into the same folder (subfolder) in either or both iCloud drive and Dropbox.

Can you move the pictures folder to be inside your Dropbox folder on your Mac?

Automating the same files in two sync services is going to eventually cause corruption and/or data loss. You'd be much better using one sync service them make use of a local backup such as TimeMachine or CarbonCopyClone (or both if you're paranoid) to external drives.
