Been having this issue for a long time.

I connected iPhone SE with Photo app on Mac yesterday and imported all the new photos. Today, I connected the two devices again and sure enough some photos (not all) shown up again as new items.

I don't understand why it's behaving this way but this happened in the previous MacOSs too, I just thought to post it here.

Anyone with similar issue? Any fix?

I just came here to ask about the same problem. I went on a very long trip and took many thousands of photos, all or almost all of which were imported to Photos via Photo Stream. But now, when I connect my iPhone, it wants to import all or almost all of them again. What's the best way to deal with this without either losing photos or importing masses of duplicates?

I'm hoping that if I import to a folder via Image Capture and then manually import them to Photos, they will be recognized as duplicates. Is there a better way? Thanks.

Well, that isn't working. The problem seems to be that Photos sees JPG and HEIF versions of the same photo as different photos for importing purposes. In the Photos control panel on my iPhone, I had "Transfer to Mac or PC" set to Automatic instead of Keep Originals, which apparently meant that photos were sent to iCloud, and then imported to Photos via Photo Stream, in JPG format. Afterward, when connecting my iPhone to my MBP, Photos sees HEIF versions and wants to import them as new. Not good.

Even with "Keep Originals" now selected on my iPhone, it is still uploading JPGs to iCloud, which are then imported via Photo Stream on my MBP. What a mess.
