If I select an image, then choose File > Print, I have some choices for the printed output, such as page size, fit or custom.

But I would like to take a portrait photo and print it on a smaller (4"x6") with a selected / cropped middle section that is in landscape. I don't actually want to rotate the image - the selection will be made in landscape dimensions.

Can anyone help?

Only option is to duplicate the photo and apply the changes to the duplicate only?
 
Use the crop function before printing. Select roughly the area of you picture, adjust the ratio to 3:2, move the picture content within the crop frame as required, print.
 
The whole purpose of the Duplicate function is to allow you to make alternate versions of the same image - different crops, B&W or color effects, etc. If you don't need to save the alternate version(s), then delete the duplicate after you print.

The Print function is not meant for editing images - it's meant for printing the images you have already edited. It's no different than using a Word Processor - you wouldn't use the Print dialog box to change the page layout in any meaningful way (I know, you may have fit-to-page options, but it's still better, when possible, to design your page layout before printing - if you laid-out a brochure in landscape orientation you wouldn't try to print it in portrait).
