On my work computer I can't ever link a layer mask to the image it's masking (the option is unavailable in the Layer menu). On my home computer, however, they are linked by default; I can link and unlink at my whim.

I don't believe I have them set up any differently, but obviously I'm missing something. How do I make the option to link layer masks available?

I am experiencing a similar issue. On my laptop (CS3) I can't transform the layer mask and the layer at the same time, it seems as though the chain icon is nowhere to be found.
 
It is. I realized that later and ended up finding that Help article later on. In truth, I forgot I ever posted this.

I really don't like that you can't link a layer mask to a smart object, but the workaround (putting the smart object in a folder and applying the layer mask to the folder) is workable.

I found that you can also just convert the entire layer into another smart object. That way you can move it around as if they are linked.
 
Yep, that is definitely a workaround. Another, more expensive workaround is to upgrade. You can link layer masks to smart objects in CS5 (and maybe CS4, I've not really used that version much).
