photoshop: how to

How does one create something like this from scratch using photoshop?

I'd start this in Illustrator. Draw and space out the dots mathematically there and subtract them from a huge square. Then import this new shape into photoshop. 1st layer is a plain black background, 2nd layer is the imported shape which use use the emboss filter on. Mess about until it looks just right then add a gradient mask on top to get a highlight effect and bobs your mothers brother.

In Photoshop, I'd create a small pattern then use pattern fill.
