This problem is getting worse and worse for me lately. Without having installed or changed anything very substantially.
Immediate fixes are; cmd-tab to change applications, and move up and click menu bar.
I've tried a few suggestions I found online; e.g changing cursor size.
It's a nuisance.
I can't find anything on the net to solve it.
Only clues I have:
Only since Catalina.
More since I enabled Sidecar.
More when I run Photos, Lightroom. (Fullscreen)
Often when changing spaces.
But these are only suspicions.
Might have to do with having many displays/fullscreen?
Main display is not my imac, but a TV via HDMI?

Imac 2017, 10.15.4, two external monitors + occational iPad sidecar, separate spaces.
I usually use 3-6 fullscreen spaces.
I might be pushing the graphics processor/software?

Thoughts, experiences?

But do you have a reason to beleve that it will fix the problem?
I'm not a believer in upgrades fixes things.
I just did a restart, first in a couple of weeks, and things seem better.
But I suspect that the problem will re-appear, as I begin to stress things. I'll report back.

I have seen issues resolved by applying the combo update. It is worth a try in your case.

A couple of other things to try, boot into Safe Mode and see if the issue happens. This could give you an indication that some third part software might be causing the issue.

Try creating a new user account and see if the issue happen in the new user account.

This is kinda generic advice.
I prefer just tagging along with my system which is perfectly functional, except for this pointer problem, and see if I can learn something if it shows up again. A system update might introduse new variables.

I'm assuming you know if you shake the mouse rapidly the curser's size is exaggerated? I had the same occurrence this morning when viewing a pict in Photoshop. Shook the mouse, and there it was.

Yep.
Doesn't work.
Only cmd-tab or (invisably) move cursor to menu bar and click.
