I purchased several HomePod Minis to use around our house. Our household consists of two adults and three children with their own Apple IDs. I was excited for each of us to be able to use "personal requests." I was quickly disappointed when almost every time any of us asked "Hey Siri, who am I?" Siri responded with "I'm not sure who's speaking." I scoured these forums and many other online resources. I turned every possible option on and off, I removed and re-added each HomePod to my home app. Nothing worked. I finally called into Apple and we found a solution. Hopefully this helps someone else with the same problem.

Note: Removing the HomePod and resetting the device through the Home app is NOT a full factory reset. Full factory resets are performed by removing the accessory and resetting it within the Home app, unplugging the device for at least 10 seconds, plugging it back in, waiting a few seconds for the top display to start spinning, and then holding your finger down on top until you hear three beeps (which can take ~30 seconds).

Here is what I did:

1) Removed each device from my home in the Home app
2) Factory reset EVERY device (see above)
3) Created a NEW home in the Home app (not inviting the others in my household yet)
4) Made sure each person in the household had the following turned on: iCloud Drive, iCloud Keychain, iCloud Siri, Siri & Search "Hey Siri", and Siri language as English (United States). (In my case, not every person had iCloud Siri and Keychain turned on.)
5) Added one HomePod to my new home - one at a time. Each time asking "Who am I?" to make sure it was working for my account.
6) Once each HomePod was in the "new" home and personal requests were working correctly for me, I invited each household member (one at a time).
7) As I added each family member to the new home, I had them individually test "Who am I?" And each time, it worked!
8) I deleted the old home.

Fortunately, they are now working correctly almost 100% of the time. The two steps I didn't find in other threads were steps #2 and #4. I don't believe creating a new home is necessary for everyone, but these were the exact steps I followed. Performing an actual factory reset and turning on the correct settings in iCloud are likely what I was missing. You also need to make sure "personal requests" are toggled on within each HomePod and you have adjusted your settings within your profile in the "Home Settings" in the Home app to the HomePods you want to use personal requests on. You can also change "Require Authentication" to "Never" for a more seamless integration.

I hope this helps someone else with a similar issue.

We had problems with personal requests not working for my wife and the iPhone intercom feature constantly turning itself off. No idea what happened, but my new iPad Air arrived on Saturday and after setting it up both features magically started working across all devices. So not connected with the recent 14.2.1 HomePod update and no specific actions on our part. Just started working by itself.
