Is that just the wallpaper that has the bevels for the icons, or is that a tweak/theme?

It's just the wallpaper. I added them to the wallpaper using the ridiculously named 'Pimp Your Screen' app from the App Store. It's $0.99 or $1.99 I believe.

Do you have lockscreen clock hide? Maybe you have the lockscreen clock disabled?
