ByCon v2 is the theme, Springtomize, iBlank, and Bytafont tweaks used.

Here you go.
It's my own. There aren't really any details. Just clear off the Apple clock and bar shadows and run this LS.
Here's a zip of the LS and status. The battery needs a little work, it's a little too far to the right when draining. I don't have the time to fix it now but I can re-upload if I get around to it. The Illustrator and Photoshop files I made are included so go for it yourself if you're feeling frisky.

That's the D0ink widget I mentioned in my post, it was posted somewhere in this thread. I had to modify it to my liking though, I'm using an iPhone 4S.

Please let us know when it's available. I have to be honest, your set ups are the ones I like the best, anywhere. They are exactly how I'd like mine to look, I just don't have the know how. I'm also betting I won't be able to use any you share though as I'm on a 4s. I'm holding out for 5s or whatever it will be, when my contract is up.

you can also hide apps (for free) with sbsettings... (and even setup a hidden dropdown dock to access hidden apps...) still, springtomize is essential stuff, has a million great features...

I know there's been issues with poof, with users unable to access the hidden apps.

Love the whole old school feel and always thought it would be trick to run an iphone5 with new technology in the original iPhone's skin.

The only modification I'd make to the screen shots above is get rid of the battery percentage. The original iPhone didn't have that feature.
