Powermac G5 won't boot

My powermac g5 running tiger 10.4.6 wont boot. It will boot from install disk and utilities will repair errors (image attached) but still wont boot from start up disk. The same errors reappear when rebooting from install disk. Also ran hardware test from disk but won't boot from here either(image attached)

Replace CMOS battery. Assuming you have back ups, boot from install disk, erase and reformat the internal HDD, and reinstall Mac OS from install disk.
