i can manually close those tabs in the view that shows history across all devices but they keep popping up when I reset the phone. Any idea how to permanently get rid of this?

This has a been a known problem since iOS 8, the best way to get rid of them for sure is to close the tab on the actual device.

The funny thing is, the history for my old phone only shows up in my 6s history upon restart and not my iPad/MacBook.

I believe it will go away after 7 days of inactivity on the old device. Although I don't have a source but it has been my experience.

Is there anyway to resolve it after the fact? I can live with it, just very annoyed by it.
Removing the device from your iCloud account should do this but I'm not 100% sure. Settings/iCloud/Touch your name at the top/Devices/Your old iPhone/Remove from account
