When emailing photos as attachments the reciever reports to me that the photos are HUGE!

I am choosing the 'small' option when being asked but they are still massive looking despite being small from a Mb size wise...

I want the recipient to receive pics no bigger than 4" square and with as good a quality as possible not washed out wall size posters!

Is there a setting I'm missing?
 
If you really want a small size, you’d better edit it in some sort of third party apps before sending out.
 
Computer images dimensions are measured in resolution. The physical size is completely dependent on the screen displaying the image.

For example, if an image has a resolution of 1920x1080 displayed in its native resolution on a 65" 1080P TV will take up the entire 65". However the same image displayed in its native resolution on a 65" 4K TV will take up 1/4 of the screen.

Point is an image sizes is relative.

However your assumption is correct. In the email app you are given the options for....These are rough examples btw...

Actual : Full resolution - ex. 4032 × 3024 resolution (btw multiplied = 12,192,768 pixels aka 12 megapixels)
Large : 2448 x 1632
Medium : 640 x 480
Small : 320 x 240

So displayed on the same screen the above sizes will not only use less storage space and data but will physically appear smaller and smaller, like you want.

I can't find any setting that would alter this behavior although I'm not exactly sure what would happen sending photos in different formats other than jpeg.

I would start by sending yourself 4 emails from your phone to a PC or Mac. In each email send yourself the same image but use actual, large, medium, and small image sizes. Than on the PC or Mac look at properties/get info of the images and see what their resolution is. If they are all the same than tried resetting the phone and try again.
