Yes, this works.

Inside the box is a code. Once you get to within a month of your account expiry an option will appear to renew, within that option there is an option that says something like "use renewal code". Type in the code and your account will be renewed for another year.

Yes to both questions. Just make sure to apply the renewal before apple renews auomatically so your cc won't be charged :)

What does the "old" version mean though? Why would anyone let Apple automatically renew when you can get it at Amazon or other resellers for 1/2 the price?! :confused:

Its old stock of dot mac subscriptions. Its the same service though which is why th codes still work. Most retailers only stock the latest box editions, but other retailers can still sell older stock at a lower price - thats their prerogative. Most people don't ever use this since nobody goes out of their way to advertise them. Apple doesn't regulate the pricing for such older stock. The store is the one setting the price to clear excess inventory. Apple has already been paid. Retailers like Amazon pre-buy the software and re-sell it to you. Amazon for example, pays Apple for the boxes before selling them to you. You aren't getting the current boxing, but the code is still valid anyway.

The automatic renewal is a different function that is designed to save you the effort of buying a new box - which is what would happen if you went to an Apple store or a best buy. You get the current retail boxing. That's Apple's charging. Its a different system which rightfully is billing you at the rate that Apple charges.
