I only do this once every 2-3 years so I don't remember all the details. Sometimes I've restored from my iCloud backup and other times I've backed the old phone up to my iMac then set the new one up from that. If I remember right, the iCloud backup doesn't provide everything. Is this correct? There have been times when I've had to reenter all of my passwords and I think it happened when I set it up from the cloud.
 
If you back up to your Mac, you have the option to save passwords, and that makes the restore so much simpler.

When you backup to your Mac make sure you encrypt the backup (check the box) - this will preserve passwords and health data.
