I could've sworn that it was possible to save a website to your home screen and then treat it as an app. When it opened, it would open outside of Safari. Now it opens as a new tab. When did this go away or am I losing my marbles?

Mine still opens as if it were an app. I just tried it with Twitter. The shortcut took me to the full screen login page where as my browser tab was already signed in.

It's up to the owner of the website to enable this functionality.

You are indeed correct. It's a simple Boolean option. A single line of code:

<meta name="apple-mobile-web-app-capable" content="yes">

https://developer.apple.com/library...bApplications/ConfiguringWebApplications.html

I submitted it to Voxmedia. Since they don't have native apps, they should at least offer this option. The website still works as a website in Safari but as a web app if launched from the Home screen. Win win for everybody.
