My work has asked me to find software that will enable us to create paper surveys, run the surveys through a paper-feed scanner, and give us a page with the results.

Despite being a Mac guy, I need this software for Windows (but I'm here because I know all of you fine folks and trust your taste in software.)

Anywho - everything I've been able to find either has a horrible user interface or doesn't allow for any real flexibility in survey design.

Does anyone have any suggestions of software they've used to create, scan, and analyze surveys?

All I want out of life is:
a) An intuitive user interface.
b) Full flexibility in survey design.
c) All standard question types allowed.
d) Powerful analysis tools.
