I have set rules using contains, equals, and everything under the sun. Marked address and even created contacts for those addresses but they still going to the junk box.

I know my rules are correct because when I click "Apply Rules" they all move into the inbox.

Why are these emails still ending up in the junk box?

Yea by hotmail. But as they come through macOS mail they should be placed in the proper boxes since I have set macOS rules no? Otherwise what's the point in setting rules?

macOS Mail rules (the automatic kind) apply to messages coming into the "Inbox". If the server has already moved them to Junk then the rules have to be applied manually, as you have done.

To get the results you want you may have to log into your Hotmail account at Outlook.com and set up your rules server-side. That particular server has a decently robust rule-making system. The good thing about this is that the rules will be done before you sync any of your devices. The bad thing is you can't set up any rules that move Mail from one account to another.

One thing I'm not sure of is if you can circumvent the Spam filtering at Outlook.com. Maybe a rule up there to move all messages to the Inbox?

What's the point of rules on Mac Mail? There isn't much point to them now that we have multiple devices to deal with. I use them infrequently for "post-processing" from one account to another.
