I am setting up a custom sync between my iMac and ATV what does this selection mean/do?
 
If you leave it unchecked, you do not actually have to sync any music, movies, podcasts, tv shows. You only have to sync photos. You simply tell each tab: movies, music, etc., that instead of syncing all, you want to sync selected - and then select none. For photos you can sync all or selected albums. Then as long as iTunes is running on your Mac/PC, you will be able to stream any of your music, movies, podcasts, and tv shows on the synced library. Even on an 802.11g network. This allows you to stream everything without setting your main machine up as a streaming computer. You just have to have iTunes running. It also allows you to have access to all your media without having to decide what stuff to sync.

If you check it, then even if your iTunes is running, you will only see and be able to play content that is synced, unless you set up the same library as a streaming machine too.
