What does silence unknown calls feature actually do totally? I know it obviously silences unknown calls but are they sent to voice mail ?

The design thought behind Silence Unknown Callers is this: An "Unknown Caller" is a phone number that is not present in your contacts list. Maybe that call is unwanted (spammer, etc.), but maybe it's someone you know. Rather than automatically send everyone absent from your contacts to voicemail, this simply minimizes the disturbance when an unknown caller rings. You can then decide how to handle the call (recognize the caller - answer, don't recognize - allow to go to voicemail.).

You'd have to add them to your Blocked List to send directly to voice mail.
Are you saying blocked callers go directly to voicemail without hearing any ringing?

Is that to say that while there is no sound or vibration associated with an unknown caller call it would still come up on the phone and take over the screen as a typical call does?

It no longer hijacks the entire screen like it used to, it's more like a regular banner notification.
Interesting. Banner notification as the call comes in giving the option to answer it, or more of a missed call or voicemail banner notification afterward?

Even when you don’t have silence unknown callers enabled, phone calls are more of a banner notification now instead of hijacking the entire screen line it used to do.
Since when? Any call that I get is still as usual where the whole screen is taken over while the call is coming in.

I remember getting a phone call months ago and it was like a banner notification. I remember the sbecause I was overjoyed that the entire screen isn't hijacked anymore. I do have my cochlear implants paired as MFT devices, so that may have something to do with the way my iPhone XR displays phonemail results.
Well, that's something different. I know a lot of people have been hoping and waiting for something like that for quite a while now, but it hasn't come around. Would be interesting to find out what was behind what you experienced.

Yeah that’s not true. When silence unknown callers is on you get a banner notification saying a call was missed but normal calls still take up the entire screen.

Sadly, I use an MFi hearing aid (just one. Left ear is beyond help) and all it really does is act like a single AirPod, really. I still have to wait out the screen takeover if I get a call that I don't want to just instantly reject for some reason. And even more annoying, if I'm drawing on my iPad and a call comes in, both devices are rendered unusable. It does it even though I turned Handoff off. :(
