I'm don't recall this being an issue in IOS 4. After taking a picture in Camera App, you click the lower left picture icon which takes you to your Camera roll. After selecting the arrow option opening up the dialogue box I select message which opens up a new message with my picture...when clicking the camera icon I'm given the option to choose existing or take a new photo or video however, when you select one of these buttons you're kicked backed to your SMS message with no 2nd picture being added.

I found this issue in both the last couple of IOS 5 beta's, in the dev GM release & now on my fresh ip4s.

However, when opening up the SMS App and drafting a NEW message you are able to add multiple pictures to your message, whether from your Camera Roll or by taking a new picture.

Is this a bug?

Just tested this and the same thing happened to me. So, yeah, seems like a bug...unless Apple intended it to work that way, which seems a bit odd to me.
