I'm currently using iOS 15.3 beta, in the UK

A few days ago I received a text from a US-based colleague via SMS.
I then noticed I had a network charge for an internationam SMS to the US.

Under 'messages' I had 'Send Read Recipts' enabled, and 'Send as SMS' enabled.

I had not expected read receipts to be sent via SMS, though I am ok with them being sent via iMessage.

Was my understanding wrong? It certainly seems as if one was sent via SMS as the outbound text message is exactly the received time, and I have not sent any messages explicitly?

Has it always been this wa?

Those options in the messages menu are not related, read receipts are only sent to users that are using iMessage and it sounds like your colleague in the US isn’t using iMessage.

Depending on your carrier, some actually charge to receive messages from outside the UK and Europe, so it’s likely your carrier charged you to receive the message, rather than a read receipt being sent.
 
iPhone user in the UK, I remember a few years back I had a none iPhone (back before iPhones and androids existed!) and that used to receive SMS ‘delivered’ text messages which again I was charged for
