I bought the flashed Nvidia GTX 980 Ti 6 GB for my cMP 5,1 a couple of years ago and generally it's been a success. But there is regular glitch that occurs both within Premiere Pro 2018 and in Finder and other apps.

In Premiere the video image sometimes goes to black in the viewer and sequence monitor windows. I still see everything else - name of the file, folders, timeline, etc - but not the images from the media. This happens when there's a lot of different media in the Premiere project and the fix is to close it down and re-open it. Sometimes it will happen repeatedly and almost always after I've been working for hours. So is heat an issue?? Or RAM (I'm working with 34gb RAM in my Mac Pro 5,1 12 core, 3.3ghz on OS El Capitan).

I've switched from CUDA to OpenCL when the vision has gone to black and the image does come back but the performance of OpenCL is not great of course in Premiere.

In the other apps around the same time of the day I get these glitchy squares as in the attachment. Any ideas?

They're both set to auto update and in preferences the web driver reads: 346.03.15f02 and CUDA: 8.0.90 (GPU Driver Version 10.11.13).
 
Any reason you haven't updated to High Sierra? I see you're running El Capitan. There are newer CUDA and web drivers with that OS... Not saying it would eliminate the problem, but might be a good starting point.

Are you running Macs Fan Control? You can ramp up the fan speed for the PCI bay if you're suspecting a heat issue. Would help get some of that hot air out.

Guessing your 980 is a MVC card with Mac ROM? The GPU fan is working I'm going to guess...

Haven't yet upgraded to Sierra because of older software. Has to happen. Will check Macs Fan Control.

My experience with my MVC 980Ti (a hot little fsck'r (now side-lined, as I recently x-graded to an MSI 580)), is that I only experienced black-screen-blinks when running stock-fan-speeds (upon new installs).

I've been using iStat Menus [https://bjango.com/mac/istatmenus/] for some years, now, and (upon-install (after a new OS Initilisation)), these black-screen-blinks stop bothering me.

I would invest some time (as Alex suggests) in getting some fan-control soft running on your System to mitigate said issues.
