We just added an Airport Extreme to hopefully increase our wifi speeds (and actually stay connected which we couldn't do before). We have a modem through Brighthouse (our cable provider). Our Airport is connected with ethernet to the modem. The Wifi is turned off on the Brighthouse modem. According to Speedtest.net our download speed is about 180 but our upload speed is only 1.54. Not sure what other information to provide that would be helpful but any help would be greatly appreciated.

This is sometimes attributed to the AirPort being in Bridge Mode behind a modem that has routing capability. If possible, switch the Brighthouse modem to bridge mode if it offers routing capability and switch the AirPort to DHCP & NAT mode.

Thanks for the suggestion. It is set up as DHCP & NAT Mode.

Actually, I prefer to do it "the other way".

I have my "residential gateway" (Frontier version of "Uverse", it's a modem/router combo) set up with NAT enabled, but WiFi -off-.

On my Airport Extreme (the older, lower version) I've disabled NAT and run it in bridge mode.

Runs fine that way.

While it runs fine that way, the gateways offered by ISPs generally have poorer routing performance in comparison to the AirPort.
