Nearest I can see, if you enable the option to sync content to your ATV you cannot also stream, and vica versa. Is this true is it one or the other?

When you connect your Apple TV to iTunes, you have two choices on the Apple TV menu, one is to "Connect to iTunes" which enables syncing between iTunes and the Apple TV. You can choose what to sync using iTunes, everything else can be streamed. The other choice is "Add Shared iTunes Library". In this case everything is streamed (and only streamed).

You can only "Connect" to one iTunes library, but you can "Share" multiple libraries.

If you set the ATV to sync against your iTunes library, you can let it sync everything, or choose what you want synced and anything that isn't synced will be streamed whereas whatever is synced will play from the internal drive. This is how I have mine set up. I have it set to sync the newest stuff, but everything is available via streaming so if it isn't on the hard drive, it streams so there is a little delay but not much. I barely even think about what is on the ATV and what is streaming. Works for me.
 
Still having issues with this. When I have it set to share an Itunes Library I can stream everything but get no provision to Sync to the ATV's Drive. When I have it set to connect to Itunes, I can sync, in fact it starts syncing about 293 items and I can stop it, but find no settings to allow me to choose what to sync, or any settings to stream anything.

You choose what to sync within iTunes. Go to your Mac and find your ATV on the left side of iTunes and go under the Music tab and you can choose which playlists to sync (if any...I don't sync...just stream).
