So in order to download any file in telegram I have to have the app open when I switch to another app it stops the downlad. I have the app on gree in background refersh in general settings.

Yeah, that's up to Telegram. Let them know if you want to see it be able to download in the background - just be aware it'll become more of a battery/memory suck if it does, so they may not want to enable it.
