Does tethering a little even downgrade battery health a lot quicker?

Not battery charged level the actual health of a battery in a iPhone
 
Charging your phone more often will degrade battery health. It is the number of charge cycles that determines battery health.
 
It burns more battery so you will charge it more and that will consume cycles which will degrade your battery slightly faster.
