I use this feature daily to type and send messages from my macbook. After the upgrade to iOS9.1, it no longer works. iMessage still works if the Mac is on WiFi. But I can't send SMS text to non-iPhone users or receive them either.

In Setting->Messages, there is no longer a setting that says "Text message forwarding". GONE.

How do I get it back? Has anyone else seen this happen?

Is the iPhone signed in to your Apple ID? Look to see where it says "Send & Receive". Tap on that and make sure it's signed in.

If I sign out of my Apple ID the option to forward SMS is gone.

Well, that was part of it. It also needed email address for Send And Receive.

As much as I love the iPhone and Mac's, I hate the fact that you can't just "log in" once and be done with it. With these things, you have to log in 2, 4, 6 or more times before you have everything back to working. Now, THAT is ridiculous!
