Time Machine Full Restore question

Hi, I have updated my Mac Pro and now it won't wake from sleep, I may have to do a full system restore from time machine backup and have a question:

During a full system restore (option/R at startup) does it overwrite all files and replace them from the backup, OR does it only replace the files that have changed since the last backup?

Neither. It is boot holding Command + R keys and when you get to the Restore Partition you will have four options. Reinstalling the OS will give you an option to restore your files (from Time Machine) during the instal process.

You want command-r not option-r. But a restore will wipe overwrite whatever is on the drive with the data from the Time Machine restore point/date. So for example, if you choose to restore from a October 31 restore point, then you are done the Mac will be in the exact state it was on October 31. Any files and data added since October 31 would not be on there.
