Sounds like you are not the only one that ran into this: https://forums.macrumors.com/threads/weird-box.1953695/

It's basically one of the Zoom features that can be enabled/disabled from the Accessibility options under the General area in Settings.

(Might want to also check what your Home button shortcut is set to as well at the bottom of Accessibility settings as triple pressing the Home button could be what's behind something like that getting enabled/disabled.)

Today a strange box appeared on my iPhone 6 running IOS 9.2.1 (13D20). It floats above all opened apps and screens and can be moved around. I have now idea where it came from.

Do I need to restore? I really don’t want to loose all of my settings.

The box you're seeing like C DM said is part of the zoom features. It's the Zoom Region > Window Zoom option.

To disable just the box go to Settings > General > Accessibility > Zoom > Zoom Region > set to Full Screen Zoom

Or just disable Zoom altogether.

Thank you! -- it was in fact the assistive Zoom feature that was enabled.
