Watching tv episodes on ipad tv app, it used to show checked as seen once viewed.
For a long time now it doesn't do this unless I go back to library and then back to tv show.
On my iPad (13.5.1) from the Watch Now view, I see checkmarks on episodes I've watched. Hope you don't mind me asking but are you sure you watched from within the Apple TV+ app and not on a channel outside the app?
I also see the checkmark watching from my library > purchases too. A couple of things that might be a factor: (1) you have to watch all the way to end for the checkmark to show; and (2) if you watched it on another device, it won't show as checked on the current device until you watch it all the way as well.

I always watch till the end, but it’s only if I go back to all my library and then back to the watched show, it shows ticked.
It used to check it immediately, possibly before iPadOS 13.5.

Off the subject, but with the glut of video which is out there right on Netflix, Amazon, etc. I couldn't keep track of shows that I had watched and liked, more or less episodes.
Found the IOS TV Time app. Shows me what I've watched, upcoming episodes over all my sources.
