iPhone Xs Max, with eSIM on T-Mobile, and physical SIM on Verizon, iOS 13 Beta 4.

Each time I reboot the device, or put in into Airplane Mode (once I exit Airplane Mode), I get a message telling me "Unknown Cellular Plan Ready to be installed" (posting screen shots of messages). If I click cancel, it just does nothing, if I hit "continue" I get "Cellular Plan Cannot Be Added".

I've had eSIM with T-Mobile for several months now and physical SIM with Verizon as well, never seen this before, even with the previous builds of iOS 13. I had went back to 12.3.1 about a week ago, and then updated OTA to 13 Beta 4 and have gotten this message since.

I didn't see where anyone else had posted this happening, but was wondering if it's need for concern. I can easily go back to 12.3.1 if needed, but then that'd leave my watch useless since it's also on the WatchOS beta.

Calls/data works just fine, so no issues there either, just wondering why all of a sudden I am getting this annoying pop up, and any suggestions to fix.

I see no reason to as both lines work just fine, figured maybe it was more of a beta related glitch associated with using eSIM and physical SIM. Never had it pop up with the previous betas, they just all were too wonky for me to use for more than a day or so, Beta 4 has been working very well for me however short of this oddball issue.

Edit: I removed my eSIM (T-Mobile) plan from the device, and rebooted, and its still trying to add an "Unknown" cellular plan. I guess I'll see if anyone else is having the same issue -- again both services work just fine, so not sure where the device is thinking that there's another "unknown" cellular plan that it needs to add, as I've never had another one that is/was added.

I havent installed the beta on my iphone, just my iPad Pro with cellular, and i saw weird stuff in beta 4 re: cellular when I installed it. It didnt list my network - instead just a generic “LTE” indicator in the top bar, and the network connection would come and go. Rebooting a couple of times seemed to straighten it out, but it does suggest there is something weird going on with the cellular connectivity stuff in beta 4 since you are having your troubles as well.
 
So far I've tried going back to 12.4 then upgrading back to iOS 13 Beta 4 (tried both via the developer profile, as well as using the IPSW) and I still get prompted to add the unknown cellular plan.

I already submitted feedback and so far it shows it's open, with no others reporting the same issue, so no idea if I'm just the sole lucky one having this issue.

I am currently updating to iOS 13 Beta 3 to see if the issue is present in Beta 3 (I don't recall it being for the short period I'd tried it, but could have missed it).

Beta 4 has been rock solid for me otherwise, so having this annoying message pop up each time I reboot or toggle airplane mode (which I do neither of very often to begin with) is a small thing - as long as it's something that can/will be fixed at some point.

Will update (in case anyone else happens to encounter this issue at *some point*) the outcome of going to Beta 3.

Edit:

After updating (via IPSW) from 12.4 to iOS 13 Beta 3, I am not getting a notification that an "unknown cellular plan" needs to be installed when restarting the device and/or when toggling airplane mode. I am however getting prompted if I go into Settings -> Cellular -> Add Cellular Plan, and it's showing "Unknown - New Cellular Plan". At this point I assume it's just some sort of (that so far I'm only having issues with) bug. Going back to Beta 4 now and expect that it'll prompt me every time I reboot and/or toggle airplane mode about the unknown cellular plan.

I have your same setup (T-mobile eSim and Verizon physical sim in a iPhone XS Max), and I'm not seeing that prompt. What are your default data/voice settings?

Using TM for data and Verizon for voice/default line. Same setup I have been using for several months, no changes to plans, etc.

Don’t get the prompt on any versions of iOS 12 either. If I go into Settings -> Cellular -> Add Celluar Plan (on iOS 12) it asks me to scan the QR code or enter manually. In iOS 12 (Beta 4 and now 5) i get prompted at every restart and when toggling airplane mode. In settings - cellular - add celluar plan i get a prompt to add the unknown cellular plan.

Also makes no diference if I use verizon for data and/or tmobile for voice. Its like it sees some phantom unknown cellular plan that isnt added which makes no sense as I have not added/removed/changed either of my plans.
