I just don't want to do anything wrong and/or lose anything.

Should I back up, restore from back up, update then jailbreak?

Yeah. What I'm trying to figure out is if there's a way to restore the same Cydia apps and maybe even settings? iTunes obviously only backs up Apps you bought from iTunes, not from Cydia. I'd be OK with fresh copies of all my Cydia apps- it's just a PITA to go through and install them all again.

PKGBackup (and I believe iBye) can do what you are looking for. But with everything being so new for the iPhone 5 and 6.1 etc, there are going to be packages which are still not updated. PKGBackup can get you into trouble because you'll end up installing something that isn't updated yet for 6.1. My opinion, and I know it's a pain in the ass, but is to download AppInfo from Cydia, assuming you are currently jailbroken. With this app, you can email yourself a detailed list of all your installed app store apps and a separate email for a list of all cydia packages and repos. This way, even though it doesn't save the jailbroken tweak settings, at least you can just go down the list and install everything on the newly jailbroken device at the same time making sure everything is compatible. Sure this might take a little more time, but at least you can be confident in what you are installing.

Ok, Appinfo sounds like a good idea then. I'm on a 4S, but it is still a different OS version. I think I'll try it all on the iPad, see what issues I run into, and then maybe do the iPhone tomorrow...

Cool. Let me know if you have any questions or run into problems. I always use AppInfo before restoring just so I have a complete list of everything. I can confirm it works on 5.1.1.

So far, everything's going well on the iPad. Waiting for my 7246 songs to sync to the iPhone.
 
Both JBs went great... except that now I have like 100 apps all messed up on my iPhone, and almost nothing can be downloaded through Cydia right now, so I don't have Infinifolders, SBSettings, or anything. Guess I'll just search for all my apps tomorrow lol.
 
Awesome! Glad to hear it went well. Yea I'm having the same issue with icons scattered everywhere. I like to use FolderEnhancer to place 99% of my icons and folders into 1 main folder that I label "Apps". Outside of that main folder is where I keep the 5 or 6 most commonly used apps not including the dock. I haven't had a chance to install all my apps from the app store nor have I had time to set everything up with FolderEnhancer. But either way, these are good problems to have! hahaha

Maybe I'm just an IT idiot, but I was never able to find a fully working app that allowed it to backup all the Cydia apps, settings, sources, etc back to the way it was. I just think the way the OS and everything is, is just too complicated.

Nowadays, I just take in-phone picture of the Cydia sources/packages and send them to myself whenever I need to re-JB. It's reliable and works. Better than relying on unproven apps to expect great results.

I was very happily surprised to see that Infinifolders cleaned up my icons this morning when I installed it. There's still new apps from iOS 6 scattered around, but I'm 85% of the way there now with one installation.
