Is there an iPad app out that lets you create a form, questionnaire, template that a user could then fill out later? I was hoping to create a standard form that i could use in clinicals at school when I am researching patients.

Bento. Also, Numbers for iPad has a "form" view that's pretty handy for entering data. Bento synchs with Bento files on your computer over WiFi (also available on iPhone.)
