using Disk utility to clear HD and reformat as ExFat fails

is this just a case of having to rid of the HD? it is quite old ~5 years minimum, and like to re use it for the TV record function. I had to reformat it to Ex FAT for the tv, but after a couple of years, it failed, so tried to reformat again, autoerasing all contents, but it seems to fail to reformat. l get the screen as attached.
suggestions?

If in doubt, throw it out. Or recycle

Select the View>Show All Devices, then the top level of the disk (not the volume) and try the erase to ExFat again.
