Using on screen keyboard results in text highlighted and deleted

When I type on the on-screen keyboard often somehow the iPad will highlight my last few words, or even a sentence. I don’t notice this happening and as soon as I touch the next letter, that whole sentence is replaced.

Has anyone figured out a way to turn this off?

Does this happen in all apps, or in one app in particular? Also, whenever weird stuff like this happens the first place I would look is in Settings > General > Accessibility to see if there is some accessibility feature turned on that might make the keyboard interaction be non-standard (Sticky Keys, perhaps?).
