I'm pretty new to the web design thing and am starting at the bottom and gradually working my way upwards. My latest stumbling block is that I really cannot get my head around web forms - laying the things out is OK but in terms of making them properly interactive, I just can't fathom it. I wondered if someone would be so kind as to point me in the right direction with regards to this example:

http://www.harleysttherapy.co.uk/Survey Page.html

I put this together some time ago and was  horrified  to have it pointed out recently that it goes nowhere! So I've looked at a few scripts from other sites and I can't really get a handle on them - I've tried to 'hijack' a couple, but the scripts themselves obviously have links to other sites embedded in them because once the forms are filled out and submitted, it directs to another site.

So firstly, I gather that what I need is text file that sits on the server alongside the form layout in order for the buttons and fields to work properly.

I'm not asking for someone to create the scripts for me (it would be nice, though... :D ), just pointers as to how the code actually works (headers, footers, linking the various elements) as casual advice.

Many thanks for reading this and should you feel so inclined to help, my sincere gratitude in advance.

Kind regards
