Like a heartbeat of some kind. of course not audible when music is playing button music is really soft or a pause it is really obvious.
"Ahem cha, ahem cha". Very weird but irritating. It is via bluetooth from iMac to headphones. Apple ipod air Max and Bose OE
Anyone found the same and know how to get rid of it?

Ok I think I narrowed it down. If I connect my head phones to iPhone or other device it is not present only on my iMac 2019.
Maybe it is a bug in Mojave or hw related on the iMac.

Also is the bluetooth connection weather than on other devices since I need to raise volume quite a lot on the iMac.

Just answered my own question. Listening to my AirPods Max via the iPad gives such sound it is amazing. It is definitely my 219 iMac not being to serve up the right quality. I am using Music Match cloud so i can listen to my music on all my devices. iPad and iPodAir Max is a great combination.

What do you think is this Hw or SW limitation so it would make sense to upgrade my iMac to newer version of OS X than Mojave. I kept it at Mojave to be able to play some 32 bit games

on the second subject of your (Monologue?) I found that too with iPhones, generally any bluetooth buds or AirPods Max sound louder and better while paired to my Mac or iPad Pro compared to the iPhone, do anybody knows why? because its pretty noticeable and I've gone through every setting on accommodations and settings.

Actually said the other way around. Much better sound using the Max with my iPhone than I hear from the iMac
