The SIM cards are provided by the network you want to use it on. You can sometimes find SIMs being resold on eBay.

Depends on your provider. If you have AT&T then go to any AT&T store and ask for a sim card for the iPhone.

AT&T will only give you a sim card if you're a customer. Otherwise check eBay. I don't know of a website that sells them. It's uncommon to need one.
