MP 1,1-5,1 werid hard drive icon how to fix?

ok so let me put this into context the hard drive is a 1TB sata 2.5 it's been used in the past then a folder appears with a question mark on it then the folder appears with a question mark on it does anyone know what the problem is and how to fix it?

Boot into your recovery partition, and run a first aid to find out if your HDD might have bad/corrupted segments

If it doesn’t show any bootable disks, check your hdd connector or it could be that your hard disk is faulty. I love troubleshooting with hardware first!
