Hi as can be seen in the picture attached the very first location shown is where I work however I never typed this in to the search box it just appears and offers me up various routes and train times etc. How does it know where my work is and how does it know to show it on the right day and time etc

I feel this is very good and very handy.

Do all these locations only appear after visiting that place a number of times or should it appear after 1 time ?

Thanks

iOS maintains a list of 'Significant Locations' which can be managed via the Privacy -> System Services page. iOS will use those locations to offer suggestions in various apps.

My guess is that it will offer it after a few visits, but I don't believe it's clear exactly when a location becomes considered 'significant'. I'm not really sure how it works; as in my list I have locations whereby it states "no visits". This might be since my last iPhone upgrade etc, it's deleted data -- but I don't know for sure.

Apple only say:
Your device keeps track of the places you've recently been, as well as how often and when you visited them. Maps uses this information to provide you with personalized services like predictive traffic routing. Learn more about privacy and location services.
