Can someone tell me what these blank icons are, why they appear sporatically, why a hard restart makes them go away, and how to make them go away forever?

Im pretty sure those are services that are normally hidden but one of your tweaks isn't stable and is causing them to show up.

What jailbreak tweaks do you have installed?

A tweak is causing that.

I'm jailbroken on an iPhone 5 and haven't had that happen though it's happened to me in the past with other iPhones.

Happens to me all the time. Respring and they'll be gone again. Has happened to me almost every jb.

Yes, I get that to occasionally, just respring or reboot and they're gone, and worry no more. Happy Days.
