I was contemplating purchasing iSmartDialer from Cydia (and have no problem paying for the jailbroken app) but what happens if I need to do a restore on my phone? Will that app disappear and will I have to re-purchase it? Has anyone encountered this problem?

No, you should be good. I have two that i bought and i never have ro pay again.

I have purchased jailbroken apps, and they recognize the serial number for your phone, and can be redownloaded. At least the ones I have.

Ok, thanks! I may purchase the smart dialer then...I downloaded a free trial and really like the features (in addition to being a smart-dialer you can access/send an sms or email from the key pad...its actually a pretty neat feature). I just wanted to make sure that I could still revive the purchased app after a full restore.

Don't worry, most developers think of this very issue when making their app. :)
