I didn't do anything related to back-up but somehow this showed up. I don't have auto-update, hand-off, etc enabled either. I also have all the Photos features off, so there should be no iCloud features running in the background. In a nutshell, I didn't use my phone for a couple of hours, but "restore" showed up. It wasn't there before.

i can't figure out the circumstances when this icon appears.
but its been around at least since 2016:
 
Hmmm. Interesting.

Whatever it is, it did something significant in the background to take up 2% of battery usage.

Restore is from content downloading via iCloud (redownloading previously installed apps) or restoring content to those apps. At least to the best of my knowledge.

Also keep in mind that screen is showing the apps and services that used the battery during that time period. There will always be something there and that doesn't mean its significant usage. For example, if your battery drained from 100% to 99% something on the screen that is 100% is just telling you it used 100% of 1%. So 100% looks significant its really not when you consider the overall battery life.

The purpose of that screen is to see when the battery drops significantly you can isolate it an see which app caused it. Anything below Home and Lock isn't significant.

I find it quite strange that something had to be downloaded from iCloud or be restored to certain apps because I haven't done anything related to those.... I didn't download any apps, I didn't sync any iCloud contents (calendar, etc), etc. And I don't have apps that use iCloud services...

By chance, did you manually update your iphone or an Apple Watch on the day you saw the "RESTORE" show up? I just updated my phone and watch this morning and saw the same icon in my battery usage. Wondering if that is for the Apple Watch...
