Is there some kind of program, or upgrade thing with either a carrier or Apple to be among the first in line when the iPhones are released...? Or is the best way still just to be among the first to place an order online once they open the online ordering...?

I'm not familiar with the upgrade/trade in programs, but would like to (if possible) just buy the new iPhone outright.
 
I would be ready when online ordering begins and get yours in first.
 
The easiest way is to have your Apple store info up to date and be ready in the app or website when pre orders go live. If you are ordering through a carrier, same deal. The way I have been able to get one on release day if the pre order doesn't work out, is to check for local availability on release day if you have an apple store near you or through best buy etc.
 
After the event, place the iPhone into your favorites. That way, when pre-order goes live you get into the app. Go to favorite and checkout instantly.

Apple no longer does 3AM/12 AM pre-orders. No more staying up.

I've never bought an iPhone on launch, and plan to do it this year. Do they go live for purchase immediately after the keynote? Or a few days after?

That's what I did at the launch of the iPhone 12 Pro Max last year.

I first made sure all my credit card info was correct in my Apple account the night before since I was using a different card. Then I had the Apple website and the Apple Store mobile app pulled up in the morning at 8am.

The website wasn't available at exactly 8am... and the app was kinda slow to refresh once pre-ordering started. But eventually the app finally accepted my order after a couple of tries. I got my confirmation email at 8:15am.

Turns out I didn't need to be ready at 8am for the iPhone 12 Pro Max... that model didn't immediately sell out.

But it was still a fun experience.

So what I was worried would happen actually happened... I did not get to preorder it. Is there such a thing as being at a store when they open on Friday that would get me a good chance at one? Ordering it online now shows an October delivery date.

Why weren't you able to pre-order it when it first opened? Up until yesterday, I noticed that apple.com continuously updated their available for in-store stock (which now Apple has disabled, but will be back up again on 9/24/21). Check back on 9/24/21 for in-store pick up.

Other option would be is to just wake up early and stand in line for walk - ins.

Depends where you live! I had to stay up till the preorder went live at midnight (New Zealand)
