Other What is the carrier info needed for ordering from Apple.com

It has been a few years since I ordered. Can somebody please remind what all I need from my carrier in terms of ordering an iPhone from Apple.com. Head of Account's phone number, billing pin, etc? I have Verizon if that matters. I don't want to get to the point of ordering at 1am and realize I don't have it all and the "head of account" is asleep....

edit: I previously have done the Apple Iphone Upgrade program on one of the lines but not the other line.

Last time I did so, I just gave Apple my phone number during the purchase process. I have Verizon as well. Don’t remember giving them anything else
