It's a split for my between my Macs and my iPhone. My Macs I mainly use for productivity and working on my sites, editing videos, editing photos, etc. My iPhone is in close second though since its always at my hip and I search for everything on it. It also my social media center where I check my Twitter and Facebook. My iPad is mainly my video player and YouTube browser.
From the options on your poll it'd have to be my iMac G4. However, I think I use my AirPort Extremes more since my entire WiFi network and WiFi connection to all of my devices relies on them.
1. Work iMac.
2. iPhone. Audiobooks, public transport entertainment.
3. iPad. Couch entertainment.
4. Home iMac. Work/gaming/media center.
5. Home Macbook Pro. On the go work.
Mac and iPhone. I touch my iPad once a month just to charge it. I don't see the point of the iPad (puts on flame suit). If I'm working, I use my Mac, if I'm travelling, I use my iPhone.

To be honest, I didn't use my iPad that much until I put a keyboard on it. I don't do a lot of media consumption except for music and games. But then my MacBook Pro's keyboard broke and I was stuck.
I wish you had allowed multiple answers, because even though I clicked iPad (mini) I use my AppleTV just as much. Both get HEAVY use. I also use my Mac a lot too.
My iPhone 6 just because it's always in my pocket. It's also my main phone line. I frequently constantly it work to reference healthcare information.
I have an iPad as well which is useful, but the iPhone is easier to carry all the time. My iPad use still falls behind my MacBook use though. Having a real keyboard and real multitasking abilities makes it a far more efficient tool.
